Indexes that must be created after ensureColumns (they reference columns older volumes only gain then, or can fail on legacy duplicates). Runs the browser-aware identity swap — rows default to chromium — plus the story-file lookup index and the one-mapping-per-file unique index. Idempotent; a step that fails (e.g. duplicate mappings from before the unique index) is skipped, not fatal.
